Please fill out this form to generate a custom HTML5 page to use with your videos.

After filling out the form, you will be taken to a confirmation page. Remember to check your email for the generated HTML file. Whitelist [email protected] to make sure you receive emails. Check your spam folder if the email does not appear in your Inbox. Upload the emailed file to your course folder and freely link to it in Blackboard.

Parentheses in the placeholders means customization can occur in that space.

  1. Enter the title of your video page. This will appear in the browser's tab and page name.


  2. Enter the title of your page. This will appear in the maroon title bar on the video page.


  3. Enter the title of the video. This will appear above the video.


  4. Enter a two-word title of your video, separated by a hyphen. This is naming the HTML file:
